If you have been formally accepted to CCI and have been contacted directly by our Admissions team, there are some forms that must be submitted through our website before coming to Italy. They are as follows:

• Consent Form
• Medical Questionnaire
• Swim Skills Questionnaire
• Residence Questionnaire
• Online Uniform Order

ALL VISA RELATED DOCUMENTS AND ENROLMENT CONTRACTS FOR SIGNATURE WILL BE SENT BY EMAIL BY THE ADMISSIONS OFFICE
